Description:Primary Registered NurseExciting News: We're Expanding into New Territories and hiring a Primary Registered Nurse to serve across Winnebago, Calumet, and Outagamie, ensuring coverage and support across all territories. With the flexibility to adapt to various areas as we continue to strengthen our presence. Join us on the journey of growth and meaningful impact.The Primary Registered Nurse (RN) is responsible for the overall care of the patients and families. The primary RN directs the activities of other nursing personnel and works in conjunction with interdisciplinary team members.Schedule: 5 days per week, 8-hour shifts, minimum every 3rd weekend, 1 on-call backup shift every 5-6 weeksKey Job Duties:Provide clinical and supporting case management based on patient's diagnosis and end-of-life symptoms.Complete initial, comprehensive, and updated assessments of patient and family needs.Develop, implement and review care plans for patient and family.
