Palliative Care RN Case Manager and Educator - Everett Washington

Role has two central tasks: palliative care consultant (providing palliative expertise directly to patients and their families) and palliative clinician educator (helping RNs, APCs, physicians, and others develop primary palliative care skills). We anticipate these tasks will blend synergistically and evolve as the palliative care program grows. Primary Responsibilities: - 60%-70% Palliative Case Coordination/Consultation
- Participates in and/or leads iterative goals of care conversations with patients in various stages of serious illness, using advanced clinical communication and "whole person" approach. Care should be nuanced to disease trajectory and evolve based on disease progression; implement palliative care plans when appropriate
- Creates informative and clear documentation, particularly for patients with high complexity in need of contingency planning. Understands and incorporates best practices as it pertains to utilizing POLSTs, DPOA, and other documentation
- Proactively partners and collaborates with TEC colleagues across multiple disciplines and settings (inpatient, skilled nursing, hospice, home health, specialty, etc) to create integrative, robust care plans. Facilitates referrals to such organizations as appropriate
- Works closely with provider colleagues to ensure that palliative care case management is coordinated, particularly when patients are dealing with multiple co-morbidities, complex and/or serious illnesses, particularly towards end of life
- Conducts visits with patients and families in the Hospital
- 20%-30% Education & Facilitation to further palliative care best practices within the clinic setting
- Grows provider and staff competency in advanced practice communication skills, serious illness management and palliative care approach and philosophy of care; act as a palliative resource for teams across TEC
- With guidance from the Palliative Care Manager, provide informal and formal education for providers and staff around serious illness management, end of life care, clinical communication, effective goals of care conversations, interdisciplinary collaboration and other related topics. We will also have Resident Physicians and Medical Students rotating on our team, starting July, 2024
- Attends and participates in palliative care and related meetings, team huddles and community meetings as appropriate. Engage in ongoing professional development and learning, particularly as it pertains to serious illness management and palliative care through informal and formal educational opportunities

- Frequently must follow written and oral instructions as well as complete routine tasks independently
- Completes annual compliance training on HIPAA/Privacy/Confidentiality/Non-Discrimination/Harassment/Integrity Statement and signs Agreements
- Ensures confidentiality of patient information following HIPAA guidelines and TEC policies
- Attends training to meet requirements of the job position and as needed or mandated by TEC policies, Div. of Occupational Safety & Health (DOSH), OSHA, L&I and other state/federal regulations

- Graduation from an accredited school of nursing
- Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross)
- Washington State Registered Nurse license. Basic Life Support (BLS) for Healthcare providers (AHA)
- 3+ years of experience in a clinical setting
- Washington State driver's license and vehicle for work-related travel Preferred Qualifications:
- Master's degree or above and/or clinical licenses
- Some experience doing Palliative Care or being involved in the care of patients receiving Palliative Care or Hospice services, being part of goals of care, advanced care planning, symptom management, and family support at the end of life
- Experience with complex patients and interdisciplinary teams
