*Case Manager-Large Managed Care Company**Labor and Delivery, Postpartum and Pediatric Experience Required**Fully Remote-Field Visits are required**6-month contract with potential for extension and/or perm hire**$46 an hour**Must be located in Central Virginia**Travel will be in your local region with expense reimbursement**Must have experience working with Medicaid recipients**Must have a valid driver's license**Day-to-Day Responsibilities:* Will be responsible for performing case management activities according to the contractual requirements and in alignment with NCQA requirements. This will include potential home visits to complete assessments as well as completion of telephonic assessments, completion of integrated care plans, and conducting interdisciplinary care team meetings.
