Description: Job Title: Community Health WorkerReports to: Site ManagerSalary: Starting at $16.40/hr (salary commensurate with experience and relevant certifications)Job Summary: -- Responsible for helping patients navigate and access community services, and work closely with medical and mental health providers and case managers to ensure patients have comprehensive, coordinated, and integrated care. -- Obtain and follow-up on patient data for value-based care processes.
