Location:Northside Neighborhood ClinicDepartment:Admin Neighborhood ClinicShift:First Shift (United States of America)Standard Weekly Hours:40Summary:This Family Advocatewill work as part of Cook Children's Physician Network in our neighborhood clinics to providesupport and resources to Spanish-speaking families for issues that extend beyond their clinical needs. Duties include:

• Build individual and family capacity by increasing health knowledge and self-sufficiency.
• Responds to the social and emotional needs of our families, providing guidance and support while navigating them through the health care system and community to achieve the best health outcomes.
• Serve as both an advocate and liaison between the patient/family, health care team, and community partners.
• Functions in collaboration with the treatment team within the Cook Children's Neighborhood Clinics. Hours: Mon-Fri 8am-5pm ***This position will provide coverage to 2 clinics. All neighborhood clinics are located in Tarrant County *** Required Experience & Education:
  • 3 years' experience working in health care or with vulnerable populations required.
  • High School diploma or equivalent, (bachelor's degree in social work preferred).
  • Experience working with families and social services preferred.
  • Bilingual: Spanish & English speaking preferred.
  • Experience providing psychosocial assessments & crisis intervention preferred.
  • Strong customer service, phone, and computer skills
  • Familiarity with basic medical terminology.
  • Highly organized with the ability to prioritize and demonstrate problem-solving skills.
