We are seeking a *Nurse Practitioner *to join us at Rhode Island Hospital in Providence, Rhode Island. You will oversee the progress of a specific caseload of patients across the care continuum, including pre-admission and post-discharge. You will also act as an educator/consultant and liaison to healthcare providers caring for patients.*Responsibilities*: - Manage a caseload of patients in partnership with the healthcare team - Oversee care coordination and provide direct care in acute home care and ambulatory care settings - Demonstrate advanced physical and psychosocial assessment skills and knowledge of family dynamics - Perform advanced practice skills under the supervision of the Medical Director - Participate in the education, clinical research and mentoring of healthcare providers - Facilitate patient/family education in area of expertise - Annually complete mandatory safety infection control, BLS and ACLS/PALS*Qualifications*: - Master's degree from an accredited nurse practitioner program and bachelor's degree in nursing - Two years of experience as an RN in acute care hospital setting required; 1-2 years of experience as a nurse practitioner preferred - RN licensure in the state of Rhode Island - Basic Cardiac Life Support (BLS) and specialty certification required*Working at Lifespan*At Lifespan, we utilize a multidisciplinary team approach to medicine and provide the right blend of autonomy and collaboration.
