Patient Navigator is responsible for implementing the comprehensive care plan to each patient as determined upon enrollment in the program.Your Bachelor of Science in Nursing preferred or equivalent professional experience Clinical Nursing and Case Management Experience Preferred Licensed in Texas as a Registered Nurse.
