*Job Title:*RN - Team Coordinator (FT) Nights A-4 Post-Surgical Acute, HVMC - Kingsport, TN*Job Code:*MSHA000BPJ*Location:*HOLSTON VALLEY MEDICAL CENTER, Kingsport, TN*Position Type:*Full-time (scheduled 72 hrs or more per pay period)*Job Definition**Job Description**SCOPE OF POSITION*The RN Team Coordinator, under the supervision of the clinical nursing leader or department manager, is responsible and accountable for the care of the assigned patients and directs others to assure compassionate and patient centered care is completed on a shift to shift basis. The RN is expected to practice autonomously within the scope of professional standards, the rules and regulations established by the Tennessee or Virginia Board of Nursing, and knowledge of the policies and procedures within Ballad Health. The RN shall demonstrate skill in developing and guiding collaborative collegial practice that delivers safe competent care based on meticulous application of the nursing process. This individual shall demonstrate sensitivity and understanding of the diverse cultural backgrounds of patients and caregivers. The Team Coordinator will work directly with staff on the unit to ensure compliance with patient care and service problem resolution. The position involves maintaining current knowledge of evidence based nursing practice and the ability to demonstrate critical thinking skills in the implementation of all phases of the nursing process. The RN will actively participate in patient care quality and performance improvement through data collection, aggregation and analysis of relevant key indicators of quality (KIQs). The RN will participate in LEAN initiatives for the improvement of processes and patient care delivery. This commitment to quality results in the development and implementation of action plans to include the monitoring of the effectiveness of improvements. The RN must have knowledge of nursing processes and evidence based practice that constitutes the care given in an acute care hospital setting. The RN will serve as a professional role model as a caregiver, mentor, teacher and resource for peers and other providers. This individual will demonstrate competency requirements for the Registered Nurse in employed clinical department and for the specific patient population served. The RN is responsible for the nursing care of patients assigned to him/her.The Team Coordinator shall assist with patient assignments, planning, organizing, evaluating, and supervising nursing care within the department. This individual will facilitate communication within the department and interdepartmentally. The Team Coordinator will identify conflict at the unit level and initiate resolution and/or escalate through the chain of command as indicated. The Team Coordinator will provide direction and support for nursing staff. The Team Coordinator shall serve as role model for clinical and ethical practice within the department. The RN is responsible for education and implementation of patient care and department/organization initiatives. The RN will plan and conduct a minimum of two hours of job related educational programs. The RN will verbalize understanding of Shared Governance Model and willingly serve on at least one Shared Governance, hospital committee, task force, or project to improve patient care or work area.Individual must possess excellent interpersonal communication skills and function as a liaison between physicians, interdisciplinary treatment team, patients, and families. The person in this position must be able to work as a team leader by communicating effectively with team members and providing an open dialogue for solutions. It is of the utmost importance that written communication is legible.This individual will serve as resource and clinical expert for mentoring staff, team development, and nurse retention. The RN is responsible for mentoring new staff and new graduate nurses either individually or through an assigned preceptor. The Team Coordinator will monitor and report to clinical leadership trends that might indicate an individual team member is having difficulty performing role expectations. The Team Coordinator is expected to set an example for all team members of commitment and loyalty to Ballad Health by openly supporting and participating in hospital projects and events.The person must be committed to continuing education by staying abreast of current medical/healthcare information and by applying evidence-based practices in areas of responsibility in order to remain competent in the ever changing role of the position and development in health care.The RN will serve as a liaison to System Clinical Educators ensuring new clinical processes are adopted and provides feedback to clinical education if concurrent training requires revision or additional support. This individual shall teach colleagues/peers and serve as an effective role model is a basis of professional practice. The Team Coordinator will demonstrate compliance with established Ballad Health policies, dress code, and standards of professional conduct. Confidentiality is essential to the role. This individual will assist the department leader in interviewing and selection of team members, and provide input into competencies and evaluation of team members.*REPORTING RELATIONSHIP*See Table of Organization.*EDUCATION AND EXPERIENCE*College degree required. BSN preferred. Health care related Bachelor's degree or associate nursing degree required. Preferably the RN with an Associate Degree will be actively pursuing a bachelor's degree from an accredited school.Minimum of two years of clinical experience as a RN is required. One to three years of experience in an acute care setting preferred. A relevant background in the clinical position applying for is preferred. Supervisory experience preferred.
