  Shift Timings: Day time. 8 am - 5 pm with 1 hr for lunch (hours flexible), weekly meeting 1 hr, every 4th weekend
  NOTE: Need to use their own vehicle
  Required: 2 years RN-Home Care experience with OASIS documentation.
  Job Description:
  • Conduct initial patient assessments to develop individualized care plans tailored to the patient's needs and preferences.
  • Provide skilled nursing care, including wound care, medication management, and symptom management, in the comfort of patients' homes.
  • Educate patients and their families on disease management, medication administration, and other aspects of care to promote independence and improve outcomes.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate comprehensive care for patients in the home setting.
  • Monitor patients' health status and response to treatment, adjusting care plans as needed to optimize outcomes and ensure patient safety.
  • Document all care provided accurately and timely, maintaining thorough and up-to-date patient records in compliance with regulatory standards and agency policies. Required Credentials:
