RN Case Managers facilitate communication and care coordination along a continuum through effective transitional care management. They focus on the progression of care by completing daily medical reviews and screening patients in accordance with CMS conditions of participation. They help to manage hospital capacity and arrange post-acute services for patients upon discharge and while doing so support the patient's right to self-determination, and recognize the significance of social determinants of health and complexities of care.
