In this rewarding role, you will provide high-quality care to patients in the comfort of their own homes.Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, wound care, and other treatments as prescribed.
• Educate patients and families on disease management and self-care strategies.
• Collaborate with physicians, therapists, and other healthcare professionals.
• Maintain accurate and detailed patient documentation.Qualifications:
  • Active and unencumbered RN license in the state of Oregon.
  • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN) preferred.
  • Open to candidates without direct home health experience will train. Long term care background is a plus!
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Reliable transportation and a valid driver's license.Benefits:
