Elwyn provides behavioral health services to improve the quality of life for individuals living with mental illness and addictions, serving over 7,000 individuals across 6 states. Elwyn Adult Behavioral Health Services is seeking a compassionate and dedicated Case Manager to join our dynamic Assertive Community Treatment (ACT) Team in Raleigh, NC. This isn't just a job - it's an opportunity to make a real difference in the lives of others. You'll provide critical support, coordination, and resources to help individuals with serious mental illness and co-occurring disorders achieve their recovery goals and live more independently. Established in 2007, our ACT program has a proven track record of success in helping individuals with severe and persistent mental illness achieve greater independence and integrate back into their communities. We serve up to 120 individuals in Wake County, focusing on reducing reliance on institutional settings while fostering self-sufficiency. $42,000 - $47,000 / year, comprehensive benefits Monday - Friday, 8:30 am - 4:30 pm What you'll do: - Advocating for Clients:You'll be a champion for your clients, ensuring their voices are heard and their needs are met. Working collaboratively with the ACT Team, you'll develop individualized plans to facilitate recovery and empower clients to reach their full potential.
- Building Strong Relationships:You'll develop strong therapeutic relationships with clients, creating a safe and supportive space where they feel comfortable discussing challenges and setting goals. Your ability to connect and engage effectively will be crucial in their journey towards recovery.
- Utilizing Evidence-Based Practices:You'll employ evidence-based interventions, including the PRISM model and Cognitive Behavioral Therapy (CBT), to deliver effective support to your clients. By staying current with best practices, you'll ensure clients receive the most up-to-date and effective treatment approaches.
- Providing In-Vivo Care:You'll be directly involved in supporting clients in their daily lives (in-vivo care). This includes tasks like medication monitoring and education, providing psychoeducational activities, and helping clients develop skills for daily living (ADL/IADL), community integration, social interaction, and employment.What we're looking for:
