Safire Rehabilitation of Northtowns is currently seeking a full time. Resident Care Coordinator. The primary purpose of this job position is to assist in the planning and implementing nursing care of residents within our skilled nursing facility. The Resident Care Coordinator will be Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members. He/she will participate in resident and family teaching and education. He/She will further maintain a role as a resident advocate with a focus on the facilitys mission of providing superior care and excellence.Responsibilities:Conduct daily rounds to monitor staff compliance with standards, regulations and the companys policies and procedures related to the provision of resident health and personal care services.Develops and implements an annual training calendar based on identified needs to promote increased competency in direct care personnel.Participates in the pre-admission/admission/re-admission process as assigned by the Executive Director.Monitors for and immediately reports unsafe/hazardous conditions/equipment in accordance with established facility policies and procedures.Qualification:Must possess a current, unencumbered, active license to practice as an Registered Nurse in the state of employmentMust possess, as a minimum, a Nursing Degree from an accredited college or universityMust have, as a minimum, two (2) year of recent experience in a supervisory role in a nursing care facilityOrganize, develop and direct the overall operations of the Nursing Service Department in accordance with current federal, state and local standards, guidelines and regulations that govern the facilityWork directly with the Executive Director and the Medical Director to ensure the highest degree of quality of care is maintained for each resident at all times.Shift & Wage:Full-Time Opportunity AvailableShifts Needed: Days Monday thru FridayBenefits and Perks:Eligible for benefits the first of the month following 30 daysComprehensive benefit packages including medical, dental, and vision401K through FidelityPaid Time Off (PTO)Educational and tuition opportunitiesVarious Discount Programs OfferedEmployee Recognition ProgramsCulture of employees creating an IMPACT!ABOUT US:We are a premier skilled nursing facility located in Erie County, New York.
