We strive to provide QUALITY services to our patients.Reporting to the Coordinator Of Care (COC), the Field RN assesses, plans and coordinates home care services for assigned patient caseload and provides skilled nursing care in accordance with professional standards, regulatory guidelines, meeting agency standards, and obtaining appropriate certification.Duties and Responsibilities Perform patient assessments to implement the best plan of care Coordinate resources & services for assigned caseload; working with the patient, family & physician Maintain up-to-date/accurate documentation for assigned cases Maintains a cooperative relationship among health care teamsJob Qualifications Required Active and unrestricted NYS RN license Excellent written and verbal communication skills Ability to meet tight deadlines.
