The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn for over 40 years. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.We are seeking a Behavioral Health Care Manager to join our team. Behavioral Health Care Manager is part of the team concept in the Center's health delivery system, which is devoted to the delivery of integrated behavioral health and primary care in an ambulatory setting with emphasis on prevention and health maintenance. The Behavioral Health Care Manager assists the Nurse Practitioner, Licensed Clinical Social Workers (LCSW), and other clinical staff in performing his/her duties and carrying on the necessary functions required in accordance with established BSFHC policies and procedures. This position involves working closely with the Behavior Health Team implementation strategies and aimed at encouraging patient retention and reaching out to past patients to encourage re-attendance at our center and any policies thereof to achieve BSFHC's mission.Bilingual Spanish a mustESSENTIAL JOB RESPONSIBLITIES AND DUTIES:Duties and Responsibilities include but not limited to:--- Thoroughly identifying the needs of referral sources, demonstrating a clear understanding of how referral sources make decisions and identifying decision-makers.--- Improving relationships between referral sources and Medical Resources Home Health.--- Educating physicians, hospitals, skilled nursing and assisted living facilities, and adult foster cares regarding the regulations of the home health industry, Medical Resources Home Health policies and our scope of service.--- Informing physicians and facilities on patient summaries of care.--- Serving as a liaison between Medical Resources, physicians, and other referral sources in the community.--- Facilitate and manage referrals, prior authorizations, and front desk office--- Daily documentation, strategic goal-planning and problem-solving.--- Notes brief history or the reason for visit in the patient's EHR;--- Generates follow-up appointments and internal referrals, and prints visit summary at checkout prior to their departure;--- Assists with generating external referrals upon patient discharge to improve patient flow;--- Provides ongoing patient education about depression and treatment;--- Engages patients in behavioral activation or pleasant events scheduling at each contact;--- Refers patients for counseling or psychotherapy;--- Notifies the PCP when the patient has been in treatment for more than 10-12 weeks without adequate improvement;--- Coordinates consultation from the psychiatrist regarding treatment changes;--- Follow up of care transitions progress, identification of high risk members requiring specialized complex and collaborative plans of care with behavioral providers, medical providers, community agencies, and outpatient case managers. Tracks treatment engagement and adherence using a registry.--- Reaches out to patients who are non-adherent or disengaged; reaches out to patients in acute treatment as well as patients who have improved and are monitored monthly to ensure that they are stable before being graduated from active care management;--- Will participate in the development of existing programs and grants to manage and achieve improved outcomes for high-risk members.--- Performs regular analysis (weekly, monthly, quarterly and yearly) as well as ad-hoc requests that identifies actionable patent outreach opportunities and policies thereof;--- Attends meetings as required (Staff, Department, In-service training, etc.);--- Any other duties or responsibilities the clinical staff may assign from time to time and when the need arises.EDUCATION/EXPERIENCE REQUIREMENTS:--- BA/BS Degree or BSW--- Minimum of 2 years experience in behavioral health care settingsOther Related Skills/Experience and requirements:--- Excellent with Microsoft Word, Excel Spreadsheet, and other computer applications and information systems.--- Knowledge of eClinicalWorks (eCW) system and electronic health record--- Experience working with disadvantaged populations helpful and knowledge of health disparities highly desired.Salary - $20-$25Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical, 401k Retirement Plan with discretionary Match, Free Life Insurance and Long-Term Disability, Transportation Plan, Generous Paid Vacations and Holidays.BSFHC IS AN EQUAL OPPORTUNITY EMPLOYER More...
