- Provide comprehensive advanced nursing care, including physical examination, history-taking, emergent interventions, and management of chronic conditions, while ensuring adherence to standards of care
- Collaborate with patients, families, and healthcare professionals to assess, plan, and manage care across various settings, including in-home assessments and monitoring changes in health status
- Ensure compliance with regulatory bodies and quality measurements
- Educate patients and caregivers on chronic disease management, self-care, and appropriate responses to changes in health status, while providing support and guidance to interdisciplinary team members
- Manage medical record documentation, billing information, and participate in committees and advisory groups related to quality improvement and professional development Qualifications
