As a TBI Service Coordinator, you would be responsible for: ---Providing service coordination to individuals and families residing in the community at large---Maintaining current knowledge base of DOH policies, procedures and regulations related to service coordination---In general, the services entail helping individuals and families access appropriate programs, benefits and other opportunities that improve their quality of lifeTBI Service Coordinator Minimum Salary: $20.51/hourTBI Service Coordinator Maximum Salary: $23.07/hourTBI Service Coordinator Schedule: M-F 8:30am-5:00pm; Combination of in-office and remote workQualifications:---Valid NYS Driver's License---Experience is required. You must:o(A) Be an individual with a Master's degree and one (1) year experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o(B) Be an individual with a Bachelor's degree and two (2) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o(C) Be an individual with an Associates degree and three (3) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; ORo(D) Be an individual with a High School Diploma or equivalent (GED) with four (4) years experience providing Service Coordination and information, linkages and referrals regarding community-based services for individuals with disabilities; OR o(E) Be an individual who has successfully served as a Regional Resource Development Specialist for one (1) year in the HCBS/TBI Waiver Program.
