Case Manager Job Locations US-NY-Bronx ID 2024-4982 Category Senior Living Housing Position Type Regular Full-Time Overview The Case Manager role at 1880 Boston Road Apartments located in the West Farms neighborhood of the Bronx, will holistically address client's medical, social and community needs in an effort to reduce the cycle of avoidable hospitalizations and over utilization of emergency departments. Under the supervision of the Social Work Supervisor and Director, the case manager will assess monitor and provide services to medically frail and formerly homeless seniors. Responsibilities The case manager will be responsible to implement the program's policies and procedures which create a case management system that provides for initial and ongoing assessment of a tenant's strengths and needs, oversees and coordinates an Individual Service Plan (ISP), crisis management/intervention, provides or arranges for the provision of identified services/resources, monitors the receipts of such services, evaluates their effectiveness in improving tenant functioning an quality of life and documents the activities and outcomes. Responsibilities also include providing tenants with as needed emotional and social support, education on entitlements benefits and assisting with maintaining benefits. Qualifications Bachelor's Degree in Social Work or in a healthcare related field or its equivalent from a four-year college (BA/BS/BSW) plus one year of related experience; or Associates degree (A.A.S.) or equivalent from two-year College or technical school plus three years related experience and/or training or equivalent combination of education and experience. Formal education must have included completion of a course of study in one of the following areas but not limited to: social work programs, gerontology studies, human services for dependent adults, human resources or home economics. Related work experience may include but need not be limited to residential care, rehabilitation, health care management and social work. Compensation: $50,000 min. to $60,000 max.
