The Clinical Screener will have responsibility to assess and clinically evaluate referrals for clinical appropriateness for admission to the Lawrence Rehabilitation Hospital. The Clinical Screener will communicate directly with admissions/intake and clinical and administrative leadership at the Lawrence Rehabilitation Hospital to support the admission process via the completion of a clinical screen, identification of applicable equipment needs, and reviewing of medications to ensure sound fiscal decision-making around acceptance, enter clinical assessment into UDS IRF platform, while ensuring expedient response to referral sources and optimal care transitions. Manage clinical qualification of referrals received by hospital accounts with the goal of expedited response to referral sources that support conversion to admission.License/Certification: LPN, RN, SW, OTA, PTA, OT, PTExperience:Clinical licensure of the form aboveAcute Rehab experience prefferedIdeal but not required experience within an IRF and working knowledge of capabilitiesDemonstrated technical skill and ability to navigate and communicate within referral platforms, Electronic Medical Records and UDS PlatformINDHPby Jobble More...
