As a Behavioral Health Care Advocate, you will be responsible for case management of behavioral health and substance abuse cases. You'll have a direct impact on the lives of our members as you recommend and manage the appropriate level of care throughout the entire treatment plan. This is a field-based position that includes up to 25% local travel. The schedule is 8am - 5pm CST Monday through Friday. Residency in or near Omaha, NE is required. Primary Responsibilities: Manage caseload of members with behavioral health concerns Utilize advanced clinical skills to engage and motivate members via a recovery, health, and wellness-oriented approach Assist members and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services Comply with all policies, procedures, and documentation standards in appropriate systems, tracking mechanisms and databases Collaborate with providers, and internal and external teams to determine acuity of mental health concerns, barriers, and progress; coordinate appropriate services Meet all contractual requirements; collaborate early and frequently with other teams in the management of cases stratified as having intensive, complex, and high-risk needs Conduct assessments and develop plans of care to improve member health outcomes Make both member and provider contacts to address identified care plan needs. Contacts will be primarily telephonic but will include some field and facility visits Provide clinical consultation to nonclinical staff Work as part of an interdisciplinary team Meet all identified performance metrics Benefits Include: 40-hour work week.
