Job Posting Title MTM Clinical Pharmacist Job Description The Medication Therapy Management Clinical Pharmacist is responsible for activities related to Medication Therapy Management (MTM), Adherence Outreach, Opioid Case Management, and/or other clinical programs. Responsibilities -
Perform comprehensive medication therapy review sessions with MTM eligible members; review member's medication profile to identify, prevent, and resolve medication-related problems to help members maximize benefits and minimize cost; document the member encounter in the MTM platform -
Provide medication education to members including, but not limited to, medication adherence, appropriate use of medications, and minimization of side effects to improve health outcomes; document encounter in appropriate platform -
Review and discuss identified potential drug to drug and drug to disease interactions with members and health care professionals -
Contact member's provider to seek resolution on medication related problems if necessary -
Contribute to projects as needed to support business needs -
Collaborate with team members in MTM/CCM support roles by providing assistance and guidance as needed -
Other duties as assigned Minimum Qualifications -
Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree -
Current Pharmacist license in good standing with a State Board of Pharmacy -
1 year of related work experience -
Must be eligible to work in the United States without need for work visa or residency sponsorship Additional Qualifications -
Microsoft Office Suite proficiency -
Analytical and problem-solving skills with attention to detail and quality -
Ability to manage time while working on multiple projects simultaneously -
Ability to develop process efficiencies -
Ability to work well in team environment Preferred Qualifications -
MTM experience or completion of a pharmaceutical care training program -
Pharmacy claims experience -
Pharmacy Case/Care Management experience -
Experience working with Medicare members Minimum Physical Job Requirements -
Ability to work a flexible schedule including weekends, holidays, overtime, on-call, and shifts outside of Prime's core business hours of 9:00 a.m. to 3:00 p.m. -
Constantly required to sit, use hands to handle or feel, talk and hear -
Frequently required to reach with hands and arms -
Occasionally required to stand, walk and stoop, kneel, and crouch -
Occasionally required to lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds -
