The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.As we age, we often require a helping hand and medical assistance. Thome PACE (Program of All-Inclusive Care for the Elderly) understands this need and provides a welcomed alternative to nursing home care for participants. The PACE model of care allows seniors aged 55+ to remain independent in their homes and communities while receiving the integrated social, wellness, and medical care services aging adults require.What you'll do: The Registered Dietitian applies advanced dietitian knowledge and skills to provide personalized nutritional care plans for older adults.Performs nutritional assessment and reassessmentsIdentifies need and implements nutrition servicesProvides nutrition education to participants and caregiversAttends meetings and takes part in participant care planningWorks collaboratively with other IDT membersDocuments changes in medical recordsAssists in monitoring mealsCoordinates 24-hour care deliveryThe above is a summary of the position, it in no way states or implies that these are the only duties you will be required to perform. If selected for the position you will receive a full job description.What it Takes:Bachelor's degree or master's Degree from an ACEND accredited program and completed the Dietetic Internship.Passed the CDR exam and maintain Registration status in good standing.
