Description:POSITION: Psychiatric Mental Health Nurse PractitionerDEPARTMENT: Behavioral Health ServicesLOCATION: 1575 Blue Hill Avenue, Mattapan, MA 02126Reports to: Behavioral Health DirectorSALARY: NegotiableSUMMARY:The Psychiatric Mental Health Nurse Practitioner will provide direct clinical service to patients, perform medication evaluation and treatment for an assigned caseload of patients presenting with a wide range of mental health concerns. The Psychiatric Mental Health Nurse Practitioner will provide clinical support to providers and staff, be involved in other administrative activities, and help to develop programming as needed by the Director of Behavioral Health. The patients' care is client-focused, and goal oriented with a focus on improving or restoring the patient's functional level.With respect to clinical duties, core duties will include psychiatric evaluations, consultation, and treatment of referred patients; prescribing medications and follow-up medication visits; provision of psychotherapy; participation in mental health in-take conferences and in case presentations; consultation and evaluation of patients in regard to crisis situations and admission to determine psychiatric hospitalization. The treatment services provided will be based on credentialing privileges, related to areas of expertise, and following accepted guidelines and best practices. Services will be coordinated with other providers involved in the patients care including medical and behavioral health providers, and other collaborating agencies. The PMHNP will take a team-oriented approach to patient care and fully participate in integrating behavioral health care with primary care.All clinical work and documentation will meet accepted standards in conjunction with CMS, including those set by policy, NCQA, state, professional associations, and contracted insurance companies. Productivity targets will be determined on a yearly basis and may need to be adjusted based on a variety of factors including specific duties and responsibilities.DUTIES WITH ASSOCIATED CORE COMPETENCIESProfessionalismConfidentiality:Demonstrate understanding of rules, regulations (HIPAA), and clinic policies regarding patient confidentiality.Respect and maintain patient confidentiality in all aspects of care.Professionalism with patient:Treat all patients in a welcoming and respectful manner.Maintain a patient centered approach.Promptly respond to patient communications.Professionalism in context of teamwork:Wear appropriate attire in line with standards for position and clinic policy.Be punctual, mindful of attendance, communicate to others when out sick or on vacation, communicate to others if unable to attend usual meetings or are unavailable to perform usual duties.Support all team members in their work.Communicate regularly with other team members about clinic workflow issues.Maintain an open and positive attitude.Communicate with team, utilize QA, or speak with director around workflow problems identified, use urgency on issues that compromise clinical care.Welcome new employees and/or students to the team by assisting with their orientation or questions.Clinical ServicesProvide comprehensive bio-psycho-social diagnostic assessment at intake and at any point needed throughout treatment.Provide evidence based psychopharmacological treatments.Follow clinical guidelines with respect to prescribing practices. Follow clinic guidelines and protocols with respect to prescribing controlled substances.Monitor and assess for medication side effects, adverse drug events, and drug interactionsConsult with and coordinate care with PCP.If providing psychotherapeutic treatments; be well versed in treatment modalities.Provide ongoing treatment planning as it relates to psychopharmacology management.Provide crisis intervention.Use a team-oriented approach with special emphasis on communication and coordination of care.Indirect ServicesMaintain patient records that meet health center state regulations and legal standards.Complete patient records in a timely fashion.Bill services the day patient was seen.Respond in a timely fashion to PAs, prescription refills, or other clinical forms.Participate in peer review and QA activities, if warranted.Attend bi-weekly staff meetings.Read and respond to emails and EHR communications via EPIC.Conduct professional practice and behavior according to the policies and procedures of MHCH.Customer ServiceDemonstrates an understanding of customer and health care training principles by successfully completing on-line courses through med trainer.Utilizes the principles of customer service when interacting with patients/clients, team members and staff from other departments.Appropriately handles or seeks support when customer service breakdowns occur. Cultural CompetenceDemonstrates a knowledge of culture by successfully completing the on-line training on Cultural Competence.Utilizes an appreciation of and respect for diversity when interacting with patients/clients, team members and staff from other departments.Responds appropriately or seeks support when confronted with cultural biases or conflicts.Emergency SituationsIs aware that in order to respond promptly and appropriately to emergencies, any staff member may be assigned duties that differ from those in the job description or may be assigned temporarily to a different location or schedule.Understands that all staff should have a Personal Emergency Plan in place to best respond to his/her job responsibilities should health center emergency arise.Requirements:EXPERIENCE:Outpatient psychiatric care of patients; and after care of mental health patients: knowledge of prescribing medications and awareness of possible side-effects from them. Demonstrated expertise in a variety of clinical modalities. Demonstrated sensitivity working with underserved multicultural populations in a community setting. Ability to provide specialty consultation to primary care providers, and families regarding individual patients and in collaboration with primary care teams. Collaborate with the medical director in coordination of provision of psychopharmacologic treatments. Demonstrate ability to handle emergency situations in a prompt, precise and professional manner. Demonstrate expertise in non-violent crisis prevention and management. All responsibilities of the PMHNP will be consistent with the scopes of practice acts governing health care in the state of Massachusetts.QUALIFICATIONS:MSN credentialed as an advanced practice clinician in PsychiatryCurrent Massachusetts licensure authorizing practice in an expanded role as a Psychiatric Nurse Clinical Specialist; eligible to prescribe. MA RN Licensure required; CARN preferredAmerican Heart Association BLS certification3-5 yrs. prescribing in a psychiatric setting requiredA successful candidate must be a self-starter, and capable of managing staff and giving directions to others, and able to work in a highly motivated, results-oriented environment.Organizational skills and comfort working in a team-based, collaborative setting are a requirementExcellent communication skills with staff and patientsBilingual in English and Haitian Creole or Spanish is a plusFamiliarity with multicultural groups, previous work experience in a multicultural setting and willingness to remain current in the field of focus is preferredExcel, outlook, and EMR experience preferredSPECIAL REQUIREMENTS:Credentialed as an advanced practice clinician in psychiatric care; specific knowledge of prescribing psychiatric medications and their possible side effects alone and with other medications: knowledge of DSM V manual for classification of mental disorders. Dual diagnosis experience.Bilingual and/or bicultural abilities are highly desirable. Up to date immunization and annual TB testing is required. Hepatitis B vaccine and annual flu immunization are strongly recommended. All necessary vaccines are available at no charge at the health center.Results of inquiry to Criminal Offender Record Investigation (CORI) must be acceptable under health center standards.by Jobble More...
