Visiting Rehab and Nursing Services is searching for an experienced Home Healthcare Intake Specialist This is a consistent 40-hour full time position. In this role, you will be responsible for managing the intake process for home healthcare services. You will be the main point of contact for clients, caregivers and their families, ensuring that they receive the best possible support and care. You will be responsible for assessing client needs and subsequently coordinating services, as well working closely with providers and local agencies to ensure that clients receive the best outcomes possible. Your excellent communication and organizational skills are essential to this position.
