Description
Reporting to the Care Coordination Nurse Manager, the HIV Health Navigator provides outreach, risk-reduction counseling, HIV/Hepatitis C/STI screening services and linkage to care services. The HIV Health Navigator provides preventative service coordination, pre/post-test counseling and referral management for clients who are at higher risk of HIV acquisition and those who are newly diagnosed with HIV. The HIV Health Navigator is pro-active in designing and implementing workflows and initiatives to assist clients in need of services. The HIV Health Navigator builds strong relationships with clients to help get and stay engaged in medical care and adhere to their medications.
Representative Duties:Provide Direct Services for individuals at higher risk of HIV acquisition and those recently diagnosed

  • Provide education and resources on HIV prevention, and methods to access care
  • Implement PrEP navigation services that are culturally competent for clients at higher risk of HIV acquisition
  • Conduct Rapid HIV testing
  • Provide pre- and post-test counseling to those seeking HIV testing, STI screening/treatment and viral hepatitis education, or screening
  • Perform Health Navigation, to include coordination of services within the primary care team and making supported referrals to other internal and outside agencies as appropriate
  • Assist with benefits verification and completion of prior authorizations for medications associated with HIV and HIV prevention
  • Communicates with insurance companies to follow up on prior authorizations
  • Notify clients of deductibles and co-payments for HIV and HIV prevention medications
  • Communicates with clients to explore alternative medication coverage options as appropriate
  • Work with team on clinic operations to include managing and overseeing the supply ordering process, ensuring a fiscally responsible approach that aligns with the allocated budget for essential supplies.
  • Perform and complete administrative tasks in a timely and efficient manner
  • Complete documentation as required within EMR to ensure continuity of care
  • Generate and review reports within EMR to identify patients receiving medication for PrEP and ensure appropriate engagement in care
  • Assist with chart audits to identify clinical workflow compliance and collaborate with team to make improvementsParticipate in Planning and Implementation of Program Initiatives
    • Participate in program planning through interdisciplinary team meetings
    • Collaborate with clinical providers to coordinate culturally competent services
    • Attending continuing education seminars, staff in-services and staying abreast of relevant literature to maintain familiarity with trends in prevention services.
    • Assist in drafting/editing program materials
    • Assist with development/revisions and implementation of workflows to ensure medication adherenceAgency Participatory Expectations:

      • Adheres to all agency and departmental policies and procedures
      • Participates in quality assessment and improvement activities as requests
      • Adheres to the highest principles of patient and client confidentiality
      • Adheres to established safety policies, procedures and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation
      • Attends all required meetings, in-services and professional trainings
      • Attends all regularly scheduled research meetings, trainings, forums and pertinent FCH staff meetings
      • Maintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activitiesPerform related duties as required

        Requirements
        • Minimum two years relevant work experience. Note: An associate degree plus relevant experience may substitute for the minimum work experience requirement.
        • Ability to handle multiple tasks, prioritize assigned duties, and keep electronic records
        • Ability to work independently and in small groups
        • Ability to manage clinically and emotionally challenging situations with utmost professionalism, care and discretion
        • Ability to work effectively with diverse groups of individuals
        • Proficient in Microsoft office applications - Word, Excel, PowerPoint
        • Experience using electronic medical record systems
        • Ability to work harmoniously and effectively with colleagues, patients, clients and vendors across the spectrum of diversity, including but not limited to race, ethnicity, color, gender identity, sexual orientation, age, socio-economic status, national origin and immigrant status, religious or spiritual identity, disability (physical, mental, emotional and developmental), veteran status, and/or limited English proficiency.

          • Bachelor's degree with one year of related work experience
          • Experience providing HIV or substance abuse education preferred but not required.
          • Strong interpersonal communication skills
          • Experience working in an ethnically, culturally, and racially diverse environment preferred
          • Ability to meet deadlines and work within a team-based environment
          • Strong interest in LGBTQIA+ health
          • Spanish speaking strongly preferred
          • Experience with electronica data collection and databases (Access, Tableau)
            Physical Requirements:
            • Ability to meet the following physical requirements with or without reasonable accommodations:
            • Sit at a computer station for extended periods.
            • Ability to keyboard for extended periods.
            • Requires handling average-weight objects up to 15-20 pounds, and occasionally assisting patients.
            • May work with blood or blood-borne pathogens and may require OSHA training.
