- Provides case management services which support a positive treatment outcome
- Serves as therapist in IOP and continued care groups, as well as provide individual therapy as needed for SUD and/or mental health disorders
- Organizes and makes educational presentations related to behavioral health and recovery
- Develop patient-centered treatment plans based on results from the screening tools and bio psychosocial assessment
- Collaborates with medical staff and treatment team as a whole
- Consults with family members and referral sources in order to coordinate patient care
- Ensures adequate case management services are identified and addressed
- Responsible for maintaining electronic health records which include documentation of treatment activity and progress
- Ensures that patient records are accurate, comprehensive and meets regulatory standards
Therapist I (Outpatient treatment) - -Must hold one of the following types of licenses in the state in which services will be provided:LGADC: Licensed Graduate Alcohol and Drug CounselorLMSW: -Licensed Master Social WorkerLGSW: Licensed Graduate Social WorkerLGPC: Licensed Graduate Professional CounselorA minimum of three years supervised counseling experience.Strong administrative, verbal and written communication skillsTherapist II (Outpatient treatment) -Must hold one of the following types of licenses in the state in which services will be provided:LCPC: Licensed Clinical Professional CounselorLCSW-C: Licensed Clinical Social Worker - CertifiedLICSW: Licensed Independent Clinical Social WorkerLPC: Licensed Professional CounselorLSATP: Licensed Substance Abuse Treatment PractitionerLCADC: Licensed Clinical Alcohol and Drug CounselorA minimum of three years supervised counseling experience.Strong administrative, verbal and written communication skillsSUD CounselorADT: Alcohol and Drug Trainee -CAC-AD: -Certified Associate Alcohol and Drug Counselor -CAC-I: -Certified Addictions Counselor I -CAC-II: Certified Addictions Counselor II -CSAC: Certified Substance Abuse Counselor -CSC-AD: Supervised Certified Alcohol and Drug CounselorSPECIAL REQUIREMENTS: If disclosed that the staff is in recovery from alcohol and drug dependency a minimum of five years continuous sobriety is required. - -Kolmac has been a leader in Outpatient Treatment since 1973.
