We are seeking to fill this LCSW position with someone who is self-directed, flexible, and enthusiastic. Since this is a new team, the ideal candidate will be autonomous and creative, able to contribute to the development of the role based on identified nephrology patient needs. The ideal candidate will have the qualifications necessary to assist our MSW with case management in the nephrology clinic and hospital, providing feedback and support, as needed (estimated 40% effort). Additionally, the LSCW will provide support to patients receiving patient education, working collaboratively with an interdisciplinary team to provide psychosocial support, to promote self-management behaviors, and to meet the goal of early selection of treatment modality for patients with a high risk for end stage renal disease (estimated 50% effort). In addition to that, there is flexibility to further define the role, based on identified patient needs. Contents are subject to change at the company's discretion. Education Required -Master's degree insocial work (MSW). Work Experience Required - 5 years of related experience. Preferred - Experience in a medical social work field. Certifications Required - Licensed Clinical Social Worker (LCSW) as verified by the Louisiana State Board of Social Work Examiners (LABSWE) and able to engage in independent practice Knowledge Skills and Abilities (KSAs) - Good knowledge of common disease entities and disorders and medical terminology in order to understand acute and ongoing needs of patients and families.
- Excellent psychotherapeutic skills.
- Superior interpersonal skills necessary to deal effectively and courteously with patients, families, multi-disciplinary team members, departmental peers and community agency personnel.
- Advanced knowledge of an acute and chronic care medical setting, medical terminology and the psychosocial and economic effects of illness and/or trauma upon the patient and their support system.
- Advanced knowledge of managed care and public programs such as those funded by Medicare, Medicaid, Medicare Advantage, and commercial insurance plans.
- Proficiency in using computers, software, and web-based applications.
- Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
- Ability to travel throughout and between facilities and work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability). Job Duties
