This job assists with the medication reconciliation process by ensuring an accurate and complete home medication list is documented for admitted patients. Communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain an accurate and complete medication history for patients. Identify discrepancies or problems with medication history information and notify a pharmacist to seek clarification or resolution. Ensure capture of prescription benefit information and document the home medication list in the electronic health record (EHR). Education Required - High School Diploma or equivalent Work Experience Required - 2 years of experience as a Pharmacy Technician Preferred - Experience taking patient's medication history Certifications Required - Current Pharmacy Technician Certification issued by the Board of Pharmacy in the state of practice Medication History Certification from the American Society of Health-System Pharmacists (ASHP) must be obtained within 90 days of hire Knowledge Skills and Abilities (KSAs) - Must have computer skills and dexterity required for data entry and retrieval of information.
- Effective verbal and written communication skills and the ability to present information clearly and professionally.
- Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
