OverviewTo provide or arrange for the provision of services necessary to enable consumers to live, work and recreate in the community environment. The Community Support Specialist shall assist consumers with monitoring mental health symptoms, transportation, supportive relationships, and communication with other providers. Assisting also in attending appointments and obtaining medications, crisis intervention and development of a crisis plan and development and coordination of natural mental health support systems. Documentation*Complete all documentation within established guidelines.*Ability to prioritize client need and to complete required written documentation.*Demonstrate an understanding of all funder and accreditation standards.*Track and document monthly outcome objectives.QualificationsEducation: Bachelor's Degree preferred Human Services or related field. Experience: One year with population served preferred License(s)/Certification(s): Valid driver's license and insurance when driving any vehicle for work-related reasons. Knowledge/Skills/Abilities: Ability to speak, read and write fluent English. Ability to read and comprehend common terminology of the mental health field.Ability to enter documentation in an EHR and use Microsoft Office. Ability to effectively present information and respond to questions from clients, medical professionals, management, staff and the public. |
