Sets goals and objectives related to site as related to Esperanza's strategic plan. Leads the clinical team and executes policies to promote the adherence of Esperanza's clinical care model as it strives towards achievement of the Quadruple Aim. Provides primary medical care services in the ambulatory and/or inpatient settings. The Medical Director reports to the Chief Medical Officer and collaborates across sites and teams at Esperanza to advance the delivery of high quality primary medical care. Primary Duties and Responsibilities of Site Director:

  • Leads the clinical team and promotes adherence to Esperanza's clinical care model for primary care delivery as it strives towards achievement of the Quadruple Aim. Supports a team-based model of care. Implements and achieves performance goals and objectives set by the Chief Medical Officer.
  • Collaborates with Health Center Managers and other EHC leadership to ensure successful site clinical operations as related to access, quality, patient experience, and financial performance. Works with relevant Operations leadership to develop plans for improving site performance around these key deliverables.
  • Creates a culture of well-being and joy in practice. Fosters an environment where providers can thrive professionally. Facilitate high-performing teams that are positively engaged with and share commitment to delivering excellent primary care. Identifies signs of burnout and takes appropriate preventive and protective steps.
  • Recruits, screens, and recommends providers for employment at EHC. Performs onboarding, orientation, and mentorship for new providers and leads them towards full participation in a team-based care model.
  • Facilitates the high performance of the clinical care team through coaching professional behaviors and attitudes. Resolves patient complaints, conducts provider performance reviews, and mediates conflicts as needed. Institutes disciplinary procedures or performance improvement plans for providers when appropriate and in compliance with EHC Human Resources procedures.
  • Meet productivity and access expectations. Manages time off requests to ensure adequate provider coverage. Monitors schedules and caseloads of clinical providers and communicates staffing needs to the CMO.
  • Participates in ongoing quality improvement activities and collaborates with Director of Quality, Chief Medical Officer, and specialty leadership in pursuing excellence in care, including as related to UDS measures.
  • Implements and maintains compliance with EHC's policies, procedures, and guidelines to assure high quality care in accordance with HRSA, FTCA, Managed Care, PCMH, and other applicable standards. Ensure appropriate documentation in the electronic health record by direct reports.
  • Identifies safety and risk management issues related to clinical care and communicates these to the CMO. Collaborates with the CMO, Quality department, Risk Management department, and/or site Health Center Manager for any inquiry or intervention as deemed necessary.
  • Participates in the preparation of annual budget in conjunction with the Health Center Manager, the CMO, and members of the Finance Department. Shares responsibility for its successful implementation.
  • Ensures regular occurrence of administrative and clinical meetings of providers. Attends required meetings as assigned to facilitate above listed job responsibilities.
  • Performs clinical functions as described in the job description for provider in their discipline. Primary Duties and Responsibilities of Specialty Medical Director:
    • Participates in ongoing quality improvement activities and collaborates with Director of Quality, Chief Medical Officer, and site leadership in pursuing excellence in care, including as related to UDS measures. Assists and advises site leadership in execution and implementation of strategies.
    • Develops and maintains clinical guidelines based on professional society recommendations and most recent evidence base. Ensures adherence to guidelines by providers of all disciplines.
    • Provides clinical consultation and supports the Quality Director for EHC's clinical peer review process.
    • Provides clinical consultation for incident review/safety concerns if requested by Chief Medical Officer or Site Medical Director. Works with Director of Risk Management and Compliance to perform root cause analyses and update EHC's policies, procedures, and guidelines in accordance with HRSA and FTCA.
    • Assists Chief Medical Officer with recommendations on approval of clinical privileges for core skills in their specialty. May be designated to approve privileges as needed.
    • Coordinate Esperanza trainings and continuing education opportunities within specialty.Non-Essential Duties and Responsibilities:
      • Develops and maintains relationships with partner organizations and community leaders. Builds the organization's reputation through participating in community health education and public health promoting activities.
      • Collaborates with External Affairs department to assist with marketing, publicity, media relations, website content, and support members of grants team as requested.
      • Performs other duties as assigned. Requirements:Qualifications:
        • Board certification in primary care discipline.
        • Additional master's level degree or equivalent experience in public health, management, administration, or related field is preferred.
        • Basic rescuer CPR Certification.
        • Bilingual (Spanish/English) preferred.
        • Two or more years of clinical primary care experience.
        • Previous leadership experience preferred.
        • Graduate of an Accredited Medical School or Advanced Practice Provider program.
        • Two or more years of experience in a clinical or hospital setting.
        • Current IL State License, Controlled Substance.
