*Conviva Senior Care Centers* is seeking a Board Certified/Board Eligible Advanced Practice Provider for a float role in *Osceola County*, working in our outpatient geriatric primary care centers._This position requires the ability to conduct patient visits in Spanish. We are experts in senior-focused primary care with a multidisciplinary approach.*A day in the life of a Conviva provider:** See 12-15 patients per day. Opportunity to spend more time with patients* Thrive in a value-base care model and improve patients quality of life* See a variety in your caseload, from preventative care to chronic conditions, with a focus in this role on walk-in and same day, emergent visits* Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care*Your Accomplishments are Valued:** Active, unrestricted NP/PA license in the appropriate state* Current certification as an NP or PA* Current DEA license or ability to obtain a DEA license* This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program.
