SOME (So Others Might Eat) exists to help homeless and poor individuals and families of Washington, DC. Our more than 30 programs throughout the District provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare and offer the tools one needs to live with hope, dignity, and greater independence. Submission of Covid-19 Vaccination Card is required. SOME (So Others Might Eat) is currently seeking a full-time Care Coordinator (Chronic Care) for our Population Health department. Position Overview: The Care Coordinator, Chronic Care provides comprehensive care coordination for high-need Medicare members with co-morbid chronic physical and behavioral health conditions. Participates in assessment and care plan development working within a team approach to improve health outcomes, to reduce avoidable inpatient and emergency department encounters, to lower cost, and increase engagement in and satisfaction with a total experience of care. Qualifications Required: High School Diploma and 2+ years of experience in community-based health or mental health setting Preferred: Bachelor's Degree and 3+ years of community-based work; experience with homeless adults, co-occurring psychiatric and substance use disorders, Medicaid populations desired. Schedule: Monday - Friday, 8:00 am - 4:30 pm Required License/Certification: First Aid/CPR, valid driver's license to be able to drive clients. Knowledge, Skills, and Abilities: |
