JOB TITLE: Case Manager EMPLOYMENT TYPE: Full-Time, 40 hours/wk SCHEDULED HOURS: Monday-Friday 8AM-4:30PM PROGRAM/LOCATION: Eastern Region Service Center, Willimantic, CT- Community basedposition requiring travel to Willimantic, Norwich and Putnam utilizing our company vehicle PC#: 1262 ABOUT THE PROGRAM: EASTERN REGION SERVICE CENTER The Eastern Region Service Center (ERSC) is a regional case management project that is administered by Advanced Behavioral Health to assist clients who are medically indigent or eligible for or currently receiving Medicaid LIA. CHR case managers work with the staff of 5 other agencies in the eastern portion of Connecticut to provide assessments and other case management services to clients with substance abuse and/or mental health issues. Case Managers engage, encourage and assist clients in obtaining treatment and housing as well as employment, education and medical services. DUTIES & RESPONSIBILITIES: --- Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms. --- Maintains family and other collateral contacts to ensure quality, client-centered services. --- Assesses and involves natural supports in treatment planning and discharge processes whenever possible. --- Provides case management, clinical and crisis intervention supports as indicated. --- Demonstrates competency in age related or specialty issues and developmental needs for each population served. --- Uses EMR to document all services and contacts with clients, collateral contacts and appointments. --- Additional duties as required. QUALIFICATIONS: Education: Bachelor's degree in human services required. Experience: At least two (2) years experience in a substance abuse or mental health setting preferred.
