The company's founder grew up on Medicaid (public insurance) and personally knows the desperation of being unable to afford necessary medical care.*The Role:*_******This role is 70% in person, 30% remote******_Our client is looking for a reliable and passionate registered nurse to join their home health care team in the Fort Collins, CO area as an RN Case Manager. _You will help develop a plan of care and deliver high-quality, skilled nursing for the families of children with intellectual and developmental disabilities (IDD)._*In this Role you will:** Visit the homes of patients every 60 days.* Create care plans, provide care in-home to children, and oversee patient progress.* Inform parents about their childrens health and provide coaching support.* Leverage telehealth to scale the impact of your time, communicate with the care team, and interact with families.* Be the critical voice for working to help children meet great health outcomes and furthering our client's mission.*Requirements:** Must be a registered nurse with at least 2 years experience and currently licensed in the state of Colorado or hold a compact license* Experience working with and training family CNAs, preferred* *Ready to work in a fast-paced, start-up environment** Must live in the Fort Collins, CO area or be willing to relocate* Must have CPR certification.* Home care experience REQUIRED* Pediatric experience, preferred.* BLS certified, required* PALS certified, preferred*Benefits Include:** Work from home options and a stipend to set up a comfortable home office* Tools for telehealth, such as a MacBook and iPad* Paid company trips to Clients headquarters and team bonding retreats******Daily Schedule*****:*Monday:* Mandatory Nurse meeting in office in Denver 9am-10:30am* The rest of the day is admin day for nurses to work for home and get all their tasks completedTuesday-Friday:* In-person recertifications - Max 3 per day. Usually 2 recerts per day* In-person start of care assessment - Max 3 per week.
