- Participate in development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
- Perform care plan oversight of patient case load by participating in ongoing interdisciplinary care coordination
- Supervise and coordinate wound care services
- Assist with providing direct patient care for our wound care clients
- Assess wounds, determine wound care needs and frequency and communicate effectively with physician's offices, insurance and the wound care manager
- Teach and educate clients, families and coordinate care with other nurses
- Understand Medicare, Medicaid and private insurance requirements
- Maintain a positive working relationship with local wound care clinics and physicians
