Position Title: Intensive Care CoordinatorAccountability: Clinical SupervisorStatus: Full-time, Non-ExemptRevised: December 2023Hybrid Status: EE2 INTENSIVE CARE COORDINATORAlameda County Mental Health ProgramRole:Under the direction of the Clinical Supervisor, the Care Coordinator works with families referred for Enhanced Care Management (ECM) and Intensive Care Coordination. ECM is a whole person care management benefit, which includes coordinating the medical and social needs beyond clients mental health or substance use disorder (SUD) conditions. Intensive Care Coordination (ICC) includes components such as facilitating assessment, care planning and coordination of services, including urgent services for children or youth. ICC is delivered using a Child and Family Team (CFT) and is intended for children or youth whose treatment requires cross-agency collaboration. The Care Coordinator is responsible for using a collaborative and coordinated approach to linking participants with appropriate health and social services to address their specific needs and care plan goals. This role requires that the Care Coordinator is knowledgeable of relevant community resources and effective at communicating with other professionals, providers, and participants to successfully link participants to services. This position is based in our Fruitvale office, in Oakland. Starting pay is commensurate with relevant experience above the minimum requirements.Duties and Responsibilities:In collaboration with the multi-disciplinary treatment team, the Care Coordinator will assess participants to address any problems, needs, or barriers identified in these areas (but not limited to): Specialty Mental Health Services, Substance Use Disorder, Child Welfare System, Housing, Employment, Education and training.Complete assessment and care plan of participants.Provides participants with high-quality referrals and linkages for patients to necessary resources and services as identified in the Treatment Plan/Problem List, playing an active role in reducing access barriers and ensuring participants have actual access to needed services.Acts as a bridge between health and human service providers to ensure that information is appropriately exchanged, and participants are successfully linked to needed resources/services.Supporting participants treatment adherence, including scheduling appointments, appointment reminders, coordinating transportation, ensuring connection to public benefits, identifying barriers to adherence and accompanying participants to appointments, as needed.Coordinates care with other community agencies by assisting participants with eligibility, documentation, and applications, etc. needed to access services/resourcesParticipates in coordinated activities, such as the Child and Family Team (CFT).Documentation of participant progress via Electronic Health RecordQualifications:High school diploma or lived experience and 2+ years of experience providing case management or coordinated services in human services.Strong oral and written communication skillsAbility to work independently and within a teamAbility to take directionAbility to work effectively with diverse group of clients, staff, and community membersOrganized, detail-oriented, and a creative problem-solverWork EnvironmentWhile performing the duties of this job, the employee regularly works either in an office setting or in the community.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.While performing the duties of this job, the employee is regularly required to use hands and fingers to handle, feel, or operate objects, tools or controls, and reach with hands and arms. The employee is frequently required to stand, talk and hear. Must be able to reach, bend, lift and move boxes up to 15 pounds.Position Type/Expected Hours of WorkThis is a full-time position. Days and hours of operations are Monday through Friday, 9 a.m. to 5:00 p.m. Schedule defined by Supervisor and subject to change to meet the business needs.TravelLocal travel to various worksites and off-site locations are required.Work Authorization/Security ClearanceEmployees must pass preemployment clearances including livescan and TB test.
