Duties and Responsibilities: - Develops monthly panel report consisting of patients see in specified month, assigned PCP, treating provider and other relevant categories, reviews PCP assignments by resolving unassigned, new patients, and new providers.
- Ensure continuity target goals are being met and developing performance improvement plans in collaboration with CMO.
- Generates operational reports such as panel fullness, urgent/same day appointment availability, third next available, creates PCP care teams by updating staff members based on FTE hours monthly.
- Manages PCP and patient change requests and tracks patient status changes including death, transfers to other care, and makes the change in the practice management system.
- Assesses impact of requests for changes in part-time status.
- Tracks impact and supports medical and operational leaders in making decisions on cross coverage, temporary coverage, and movement of whole or partial panels tracks provider status changes (transfer, termination, sabbatical, medical leave, etc.).
- Conducts Empanelment 101 training for all staff.
- Data oversight (and governance participation) of PCP data, clinical FTE data.
- Develops internal (staff) and external (patient) communication for any changes regarding provider panels to corresponding audiences?
- Assists in developing scheduling guidelines to optimize patient access to services.
- Participates in pilots related to access and empanelment, to provide subject-matter expertise and inform decision-making within the tests of change.
- Audits of scheduling practices to ensure adherence to scheduling guidelines. Position Requirements:
