An ideal candidae has experience conducting Annual Wellness Visits. This NP should be versatile as it requires home and telehealth visits. Required Qualifications

• Current California Nurse Practitioner license
• Nurse Practitioner Certification from either the ANCC or AANP
• Current DEA licensure
• Active CPR Certification
• 1-2 years of NP experience (home care and geriatrics are a plus) Responsibilities
  • Conduct annual wellness visits daily for primary care management of patient caseload
  • Manage both acute and chronic conditions through direct intervention like medication prescription and patient education
  • Consultation with collaborating Physician as appropriate
  • Complete documentation of patient encounters, funding sources, and billing according to policy Benefits
