Your expertise will be instrumental in fostering a therapeutic environment and supporting patients on their journey to lasting recovery.Comprehensive assessment of patients' substance abuse history and prompt documentation of findingsFormulation of individualized treatment plans with measurable goals and interventionsRevision of treatment plans as needed and effective communication with the treatment teamConducting educational and therapeutic groups, providing individual/family counseling, and evaluating intervention effectivenessCollaborating on discharge plans, providing referrals, and ensuring continuity of careContributing to program development and maintaining a therapeutic environmentReporting unsafe conditions and upholding patient's rightsContinuing professional development and adherence to ethical standards and guidelinesYour Experience Matters What were looking for:Education & Licensure: Bachelor's Degree from an accredited college or university in social work or related field preferred; Masters Degree preferred.
