Registered Nurse (RN) | Home Health Location: Lynchburg, VA Agency: Cynet Health Pay: $2,267 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 4/6/2026 About the Position Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health or Hospice experience License: Active nursing license required Certifications: CPR, BLS Must-Have: Must be proficient in OASIS and HomeCare HomeBase EMR. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Must be willing to travel up to 75 miles for patient visits. Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders. Home nursing care will be based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Requires home visits and periodic office meetings. Average number of patients per day is 5-8. Each type of visit and activity holds a different point value for productivity expectations. Expected to meet a weekly productivity point requirement. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Expectation to accept every patient on the schedule, regardless of travel distance between patients. Documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation may impact billing and follow-up patient care. No guaranteed hours with this contract. Must be open to travel for floating opportunities to nearby clinics as needed. All travelers must have experience with HomeCare HomeBase and recent experience in the specialty. Local candidates are welcome. Must be available for on-call rotations as required.
