This role is pivotal in providing top-tier coordinated care, focusing on patient outcomes and effective resource management. The ideal candidate will excel in a fast-paced environment, ensuring that the care provided is not only timely but also of the highest quality. As an RN Inpatient Case Manager, you will collaborate with other healthcare professionals to assess patient needs, plan and implement comprehensive care plans, and evaluate patient responses to care. You will advocate for patients, educating them and their families about their conditions and the necessary follow-up care. Assess patient health problems and needs, and develop care plans Implement nursing care plans and maintain medical records Advise patients on health maintenance and disease prevention Coordinate with healthcare professionals to optimize patient care and resource use Manage and monitor patient medication and treatment schedules Document patient care services by charting in patient and department records Ensure compliance with protocols and overall clinical objectives Strong clinical judgment Excellent communication and interpersonal skills Proficient in healthcare management systems Ability to work collaboratively in a multidisciplinary team Knowledge of healthcare laws and regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license Certification in Case Management (CCM) preferred More...
