In this role, you will be pivotal in providing top-tier home-based nursing care to our patients, following a patient-first approach that is at the heart of all our services. This position offers the unique opportunity to work closely with patients in their home environments, helping them achieve optimal health outcomes while maintaining a level of comfort in familiar surroundings. The ideal candidate will be adept in managing various health conditions and providing the necessary healthcare guidance to patients and their families. Assess patient health problems and needs, developing and implementing nursing care plans accordingly. Maintain accurate, detailed reports and records of patient care and progress. Administer medications and treatments as prescribed by health care providers. Modify patient treatment plans as indicated by patients' responses and conditions. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Instruct patients and their families on proper home health care strategies and procedures.
