This position involves providing specialized nutritional services and therapy to our diverse patient population, ranging from pediatric to geriatric individuals. Our focus is on implementing personalized, patient-first health care tailored to the unique needs of each patient. You will collaborate with a diverse team of healthcare professionals to ensure optimal health outcomes through effective dietary management. This role requires keeping abreast of the latest nutritional science trends and effectively communicating dietary plans and advice to patients and their families. Assess nutritional needs, diet restrictions, and current health plans to develop and implement personalized dietary-care plans and provide nutritional counseling. Consult with physicians and other healthcare professionals to coordinate medical and nutritional needs; provide nutritional support for a variety of health conditions. Educate patients and their families on healthy eating habits, dietary modifications, and nutrition management for improved overall health outcomes. Document patient care and track progress towards dietary health goals, adapting plans as necessary to achieve optimal results. Stay informed on the latest nutritional research and integrate evidence-based practices into patient care.
