This role involves using both clinical nursing skills and exceptional patient interaction abilities to manage a wide range of medical cases. The ideal candidate will ensure the efficient coordination of medical services, aiming for the highest standard of patient care and satisfaction. This is an important role that bridges the gap between patients and healthcare services, ensuring that personalized treatment plans are developed and implemented in accordance with each patient's healthcare needs. Conduct comprehensive patient assessments to determine necessary medical services and care. Develop, implement, and review individual healthcare plans. Coordinate and facilitate communication between patients, family members, and healthcare providers. Monitor patient progress and adapt care plans as necessary. Ensure compliance with healthcare regulations and standards. Educate patients and their families on processes and potential health management strategies. Advocate for patient needs and ensure resource availability. Strong clinical skills as a Registered Nurse. Excellent communication and interpersonal skills.
