Responsibilities include patient assessments, care plan development, pain and symptom management, coordinating with the hospice team, and providing education and emotional support to patients and their families Requirements: •/tExperience with hospice case management required •/tExperience providing hospice care in a home setting required. •/tPrior experience with rural travel preferred. •/tExperience with McKesson/Netsmart EMR preferred CloudinfraIT Inc Job ID 294656.
