Advanced Practice Provider (APP) - Nurse Practitioner/Physician AssistantI. QUALIFICATIONSA. Education: Graduate of an accredited Physician Assistant, Family NursePractitioner, Adult Nurse Practitioner, or Pediatric NursePractitioner Program. Licensed in the State of Vermont.Certified by a relevant National Certification Examination. DEALicense appropriate for scope of practice as defined by State lawand clinic protocols.B. Experience: Experience or training in a primary care medical practice setting.If an Adult or Pediatric Nurse Practitioner, minimum one yearexperience in a clinical practice providing care to all age groups.Experience in a community based primary care practice orprogram is helpful.C. Knowledge and Ability: An understanding of the concept of community orientedPrimary care. Able to work within a team concept,maintaining positive and constructive interactions with thepublic and with staff at all levels of the organization. Musthave proficiency in computer skills including, but notlimited to email functions, spreadsheets, documentprocessing and Electronic Medical Records. Must maintaina high level of confidentiality.D. Typical Physical Demands: Requires full range of body motion including handlingand lifting patients, manual and finger dexterity and eye-handcoordination. Requires standing and walking for prolongedperiods of time. Occasionally lifts and carries items weighing upto 50 pounds. Requires normal visual acuity and hearing.Requires exposure to communicable diseases and bodily fluids.Requires working under stress in emergency situations duringirregular hours.E. Standards of Conduct: Each employee is responsible for conducting themselvesin an ethical manner, and reporting possible violations tothe appropriate channels. Employees must be careful in bothwords and conduct to avoid placing, or appearing to place,pressure on subordinates or co-workers that could cause them toviolate these standards of conduct or to deviate from acceptednorms of ethical business practice.

II. FUNCTIONS AND RESPONSIBILITIES: The Mid-Level Practitioner is responsibleto the health center clinical director and works under the supervision of a physician on a day-to-day basis.A. MEDICAL CARE1. Direct Patient Carea. Provide primary care, treatment, and health education forpatientswithin the scope of practice as defined by State law, supervisingphysician guidelines, and clinic protocols. Such treatment willemphasize continuity of care, health promotion, and diseaseprevention, and will include, but not be limited to health history,physical examination, acute and chronic care, and the prescribingand/or dispensing of required medical therapeutics.b. Document the assessment, planning, implementation, andevaluation of patient care through the use and maintenance ofproblem oriented medical records.c. Perform or order laboratory tests as per clinic protocols andCLIA guidelinesd. Review laboratory test results and other medically related reportson a regular basis, following up on abnormal results.e. Review and return patient phone calls as appropriate.2. Referralsa. Refer patients as appropriate and necessary for specialty and/orsupportive care and testing,b. Assure continuity of care for referred patients throughconsultation with medical specialists, contributing to the patienttreatment schedule and maintaining ongoing medical review ofpatient care and progress.B. EDUCATION1. Communitya. Participate in the planning and implementation of communitybased health education initiatives, including but not limited toformal and informal public presentations, screening clinics,newsletter articles, etc.2. Staffa. Participate in the planning and implementation of in-serviceactivities for health center staff.3. Personala. Participate in relevant continuing medical educational activitiesin order to maintain licensure and certificationC. QUALITY ASSURANCE1. Participate in the development, implementation, audit, and continuousevaluation and improvement of protocols, services and programsprovided by the health center.D. OTHER1. Acts as mentor to and for clinical and non-clinical staff.Encourages and participates in continual professional developmentand encourages staff to do the same.2. Works to improve clinical practice that benefits the patient, as wellas the organizational mission. Participates in Quality Assurance &Performance Improvement projects that improve patient outcomesand organizational efficiencies.3. Works with Directors/Managers/Supervisors insuringcontinued regulatory compliance and success of organizationalgoals, objectives and strategic plan.
