Hospice On Call RN needed- 4 nights a week full time with benefits! Bayada Hospice is hiring a Full Time RN, Registered Nurse for overnight On Call hospice visits ( 4 nights a week) Office is located in Colchester, VT. This office services hospice clients in territories throughout Chittenden, Franklin, Lamoille, Washington and Grande Isle counties. Prior home care and/or hospice experience strongly preferred, but not required. As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home. Registered Nurse (RN) Responsibilities:
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet Qualifications for a Registered Nurse (RN):
