Encompass Health is seeking a dedicated Pharmacist to join our team in delivering high-quality, integrated healthcare focusing on rehabilitation and home health/hospice services. Our pharmacists play a pivotal role in our commitment to help patients regain independence after serious illnesses or injuries. The successful candidate will be responsible for managing medication therapy and overseeing the medication management system. This position demands a thorough understanding of pharmaceutical care practices and a commitment to patient safety. Assess the appropriateness and effectiveness of the medications that patients are taking. Monitor patient health and progress to identify potential medication interactions and side effects. Advice healthcare team on drug dosage, side effects, and proper medication storage. Assist with the creation and management of medication therapy programs. Educate patients on the prescribed medications, including how to take them, potential side effects, and interactions. Stay abreast of the latest drug developments and regulations. Maintain accurate records of medication inventory and patient prescriptions. Strong understanding of drug properties, interactions, and pharmacy law. Ability to work collaboratively in an interdisciplinary healthcare team. Excellent communication and education ability for both healthcare professionals and patients. Analytical skills and attention to detail. Proficiency in pharmaceutical technology and software systems.
