This position involves providing top-notch nursing care to pediatric patients in a home setting, supporting them to achieve their highest levels of health and independence. As a Pediatric LVN, you will work closely with patients and their families, offering tailored care plans and fostering a safe environment conducive to healing and growth. Provide one-on-one nursing care to pediatric patients in a home setting based on the care plan. Monitor patient’s health condition, record vital signs, and communicate any changes to the case manager. Administer medications, conduct basic patient care, and oversee nutritional needs. Educate patients and families on health care needs, conditions, and options. Ensure compliance with all health and safety regulations within the home care setting.
