In this role, you will provide high-quality medical care to patients in both facility-based and home-based settings, part of our expansive network of inpatient rehabilitation hospitals and home health agencies. This position is pivotal in ensuring the health and recovery of our patients through compassionate and efficient care. Provide nursing care to patients according to the care plan under the supervision of a registered nurse (RN). Monitor patient health condition, note vital signs, and report to the supervising RN. Administer medications and provide treatments according to physician's orders. Assist with patient hygiene, feeding, and other daily activities. Document all patient interactions and treatments accurately. Support patient and family with empathy, communicating pertinent information to improve patient outcomes. Coordinate with healthcare team to deliver integrated care. Proficient in nursing practices and protocols. Strong communication and interpersonal skills. Ability to operate medical equipment. Comprehensive knowledge of medications and their effects. Skill in establishing rapport with patients and their families.
